Dependent Eligibility Verification Required for Some Employees

Employees who added dependents during open enrollment must provide eligibility documentation.

If you added any dependents to your City health plans (Medical, Dental, Vision) during the 2024 annual Open Enrollment period, you must provide documentation to Human Resources verifying their eligibility. You were not asked to provide this information during the Open Enrollment period to ensure your enrollment and changes were processed promptly. If you did not add any dependents, no documentation is required – do nothing.

Please see the attached definitions of various types of dependents and the types of documentation you can provide to verify their eligibility for your health plan(s). To avoid disrupting any new dependent coverages, please forward your document copies to Karen Heyduck in Human Resources no later than May 15, 2024. Do not send originals. Documents can be hand-delivered or sent via email, fax, or US Mail. We do not suggest using inter-office mail for your personal information.
